   • Customer Authorization Form
   • Lifeline Authorization Form
2. Complete the forms with your information.
3. Sign the Lifeline Certification.
4. Mail or Fax the completed forms to our office at (877) 293-4242 or (405) 917-6388.
5. Once the forms are received and processed, your service will be connected in 3-5 business days.
6. Please put a contact number on the form in case of any problems or questions about your service request.
7. Can't print the forms? Call our customer service department now and we will be glad to mail you the documents you need to sign so that we can start your service.
